This guideline was coordinated by Dr Maria del Carmen Casanovas and Dr Knut Lönnroth, with technical input from Dr Luz Maria De-Regil, Dr Ernesto Jaramillo, Dr Juan Pablo Peña-Rosas, Mrs Randa Saadeh and Mrs Diana Weil.
